Office InformationOffice hours are 8 am. to 5 pm. Monday through Friday. We ask your courtesy and consideration in appointment scheduling and cancellations. We always try to get you an appointment as soon as possible when you have a sudden illness or health care problem. In order to provide the best care possible to all patients, we appreciate and require 24 hour advance notice if you need to cancel or reschedule your appointment. Appointments not canceled 24 hours in advance and no show visits may be assessed a $20 fee that is not covered by your insurance. Patients with repeated no shows may be subject to termination of care from the practice. Please call if you have any questions. Insurance identification card. It is essential that you bring your insurance identification card with you at the time of your visit so we can be sure that we have the correct information for your account. If your insurance changes in any way please remember to let us know the new information and bring a copy of your new card so that we may update your account. New Patients. We ask for payment at the time of service, and require a $50 deposit towards your first visit unless your insurance lists Dr. Erde as your physician and requires a co-pay. As a courtesy we bill most insurance carriers, and will gladly refund any overpayment once we have heard from your insurance. We are unable to accept credit cards or debit cards at this time. Full payment of any amount not covered by insurance is expected with in 30 days. New and ongoing patients with managed care insurance, those requiring a primary care physician and/or a co-pay per visit, should have Dr. Erde listed as their primary care physician on the insurance card. All co-pays are due when you check in for your appointment. If you do not have medical insurance and wish to pay for your visit in full at the time of service, we offer a 10% cash discount. In order to provide quality medical care, your past medical history is important. You will receive some forms to fill out prior to your appointment. Please fill out the health questionnaire and the registration form as completely as possible at home and bring them with you to your appointment. This will allow you to spend more time with the Doctor. Due to our small office size and the extensive amount of paperwork involved, we choose not to provide service for worker's compensation injuries. If you have an injury on the job go to the emergency room or urgent care center. Prescription refills. If you need a prescription refill, please call your pharmacy with the prescription number and request a refill of your medication, even if the bottle says no refills. If necessary, the pharmacy will call us for authorization. We ask that you call the pharmacy before noon on Fridays, and allow 24-48 hours for any prescription refill.
Our LocationWe are located in the Centennial Square Plaza at 1800 Centennial Blvd., Suite #6, Springfield. From the I-105/126 freeway take the Mohawk exit and turn south onto Mohawk Blvd. Turn left (east) onto Centennial Blvd., continue through the first lighted intersection. The parking entrance is located 1/2 block past 18th on Centennial Blvd.
